This position works with children ages 3-21 creating individualized plans that are tailored to meet each child’s needs, interests and goals and works with them side by side to help them successfully achieve their goals. The Community Case Manager position is a blended role of providing case management services and community based-counseling at locations such as the home, school and other environments. This position works collaboratively with the clinical team to support the child in working toward and completing their therapeutic and recovery goals outlined in their treatment plan.
Major Functional Areas and Description of Duties
Provide individual care plan development, crisis management planning, crisis interventions, assessments, referrals and monitors service delivery;
Provide outreach, advocacy, resource identification, referral, mobilization, linkage, monitoring and coordination of services activities;
Network with family members and various providers in the community to attain positive outcomes for consumers;
Ensure that consumers apply for and maintain adequate medical coverage to enable treatment to occur on a consistent basis;
Working with children and their families to provide direct support within the community;
Document and articulate treatment interventions effectively;
Communicate symptoms, behaviors, strategies and interventions to the clinical team;
Coordinate medical services and emergency services as needed;
Participate as a member of the community support services multi-disciplinary treatment team and serve on appropriate committees, both internal and external, as determined by supervisor;
Maintain professional codes of conduct and ethics;
Perform any additional duties assigned by supervisor.
